Field notes / A practical starting point
HRT change log: keep dates and questions together
Treatment and routine notes can help you remember the context of a conversation. They cannot, by themselves, show that one change caused another.

Record the choice, not a verdict
For a treatment or routine change, write what you changed, when and your reason at the time. If the timing is approximate, say so. Keep user-entered instructions separate from your observations.
Keep instructions accurate
For HRT or any other medicine, copy only the instructions given to you and check them against the source. A record-keeping app or worksheet should not recommend a dose, interpret your prescription or tell you to stop treatment. Ask your clinician or pharmacist when something is unclear.
Give other context a place
If you choose, note other changes happening at the same time: a different routine, travel or another treatment. You do not need to capture everything. Two things appearing together in a timeline do not establish cause.
Allow “I don’t know yet”
A handful of moments is not a continuous measurement. Sparse notes, missing days and uncertain dates limit what can be compared. “Inconclusive” can be the honest summary. Bring questions to a qualified professional rather than turning a pattern into a treatment decision.
Fictional example · Not a patient story
What a small note can look like
Fictional organizational example: Recorded 18 September. Change date: approximately early September. Reason: my own note of a conversation. Existing instructions: check against the original label, not this example. Other context: a different work schedule that month. Question for my clinician or pharmacist: can we confirm which instructions apply now? No medicine, dose or effectiveness claim is supplied by this example.

Common questions
What belongs in an HRT change log?
The date you recorded the note, the actual or approximate change date, the instructions already provided to you, your reason and questions. HRT means hormone replacement therapy; this page helps with documentation, not choosing a treatment.
Can these notes show whether HRT is working?
No. A personal timeline cannot establish cause or treatment effectiveness. Keep your impressions separate from facts about dates and bring questions to a qualified clinician.
What if I cannot remember the date?
Write approximate or unknown instead of filling in an exact date. Keep the date you wrote the note separately so an older event does not look newly happened.
General record-organization information, not medical advice or reviewed clinical guidance. Contact a qualified professional for health questions. This page makes no claim about a treatment’s safety or effectiveness.
